I've used small and medium. Small puts the hook hole near center. The medium puts the hook hole in the top 1/3rd of the worm. I prefer medium.

Photo 1) Cross section with a small.

Photo 2) Worm diameter and the size recommended on the package.

Oh, and please note...you snap the pointy tip off after its inserted into the worm...just above the hook hole.

My photos show the point still intact.

I just ordered both the small and the large. We will see how they work out.

So I was able to test these out and they have positives and negatives.  First,  I did catch fish with them and they position the hook perfectly. Downside, for every fish I brought in, not only was the bait torn, the plastic holder would be ripped and I would loose the nail weight. I tried it on a senko, Geecrack's G-Crawler, a Trick Worm and a Jackall Flick Shake. They all did the same thing. I caught eight fish and went through eight weights, eight holders and eight worms.

I use the 7 mm arsenal stealth silicone tubes (they have a sizing chart on TW) with 4 and 6” senkos and super glue the nail weight, and definitely skin hook the worm.  Works great for spotted bass and skipping floating docks.

Odd.

Hasn't been my experience at all. And I consider the Bellows Stick to be fragile.

Sorry it didn't work out for you. 😥

I guess you could try a screw spring worm lock or silicone worm tube next?

I lost a couple yesterday too. I think one was a pike though because I felt a “tick”, set the hook, and came up with nothing but sheered off line.
