So that happened to my favorite jig pitching rod. 

I believe the tip top is hot glued on.  

Where can I order a new tip top?

How do I determine the right size?

First call is the manufacturer of the rod. They can advise on the specific guide manufacturer. They might even send you one.  They should be able to tell you the size needed. 
 

if they give you info but no guide, then mudhole tackle is a good place to get the tip top. Tip tops are commonly glued on with either epoxy or hot melt glue. They don’t need any more than that structurally. There’s usually a thread wrap for aesthetics. 
 

if the manufacturer is less helpful, then you can check the rod specs and then compare what you have to the pictures on mudhole. If you have a ruler you can measure the ring size (measure the ID of the metal that holds the ceramic ring) and then approximate the tube ID that goes over the blank.

Put a rubber band through the ring so you can apply tension as you apply a little heat from a butane lighter.  If it does not come off readily then stop.  If it does not come off readily with a little heat then it was epoxied on and I would recommend having an experienced builder deal with it.  Too much heat can destroy the last inch or so of the blank.  The size of the ring is not critical-anything close will be ok.  The tube should be just larger than the tip of the blank, which can be measured when the tiptop is off.

Tip tops are sized by ring/tube. Ring in mm of frame ID and tube in 64ths of an inch. Don’t ask me why. Sellers may offer mm conversion for tube size. Measure the tip 1/8-1/4” from the end to account for taper using calipers. Do not twist the top in efforts to remove it. Use heat sparingly.
