Found this laying around, just curious as to what bait it is. I’m guessing it’s made by storm but can’t find anything online.

You are correct it’s part of the Storm Wild Eye Series....its called a JERKIN MINNOW but don’t see them in stock at any US retailers except eBay. So it may be discontinued ?

The rigging is wrong. The hook should come up through the belly and out the back.
