Looks like a BPS XPS Minnow to me.

The paint job looks similar to Indiglo Halo on some of the new Duo Realis 110 SP jerkbaits.

It is a jerkbait sold at Sports Chalet.  I believe it is made by Matzuo, but I may be incorrect.  When I bought mine, they were selling them 2 to a pack; the 4" regular sized model with a 2" mini sized model added in for free.  I have that exact color, it works fine.

Well now you guys have irritated me enough to confirm that it is a Matzuo.  Specifically the Matzuo Phantom Minnow, which has been discontinued (and is why they were packaging them in a two pack).  The color is Chrome/Blue back, #200.

Bought a couple of Matzuo jerkbaits last week, bass and peacocks liked them as well as any other lure in the bag.

Matzuo Phantom Minnow. It's not very durable but has some sweet action to it similar to an X-Rap.
